Time:  Monday → Friday 8AM-4:30PM —  Appointment not necessary

 Place: Taylor Health and Wellness Center through Sept. 30th, 2016

Monroe Building (across the street from Taylor) starting Oct. 3rd, 2016 

 Offering: Flu shots! (COVERS 4 strains, most just cover 3 strains)

To members of the university community:

1.       MSU employees (full & part-time) _______________________      No cost!        

·         Paid by MSU insurance

2.       Household members of employees_______________________     No cost!       

·         Paid by MSU insurance

3.       Retirees (on MSU insurance) ____________________________     No cost!

·         Paid by MSU insurance

·         Stop at Business Office before injection

4.       Retirees on Medicare (paid for by Medicare) _______________     No cost!

·         If Medicare Advantage Plan – please check with your plan to see if Taylor Health is in network.

5.       Students       (paid by student fee) ________________________     No cost!

6.       Other_______________________________________________    $30.50         

·         Taylor does not vaccinate children under 6 years of age. If your child is under 6 contact your pediatrician.

·         Ages 6, 7, and 8 need a copy of their immunization record to determine if the patient needs 1 or 2 flu vaccines.

·         If you are on Coumadin (warfarin), must bring date and value of INR that has been drawn in the last 3 weeks equal to or less than 3.5.

·         Medicare patients must check in with the Business Office before getting their vaccine.
