In order to complete your open enrollment, you participated in a biometric screening in late 2019. What’s next?
Risk factors
If your biometrics identified a health risk factor:
- Mercy will initiate a referral to their Care Management team if the biometrics indicate that the member and/or dependent has an elevated A1C, asthma, diabetes, COPD or metabolic syndrome.
- Mercy will initiate a referral to their Mercy Health Coaching team if the member and/or dependent has two or more health risk factors.
- Mercy will initiate a referral to their Tobacco Cessation Program if the member and/or dependent discloses any form of tobacco use. Employees and/or dependents also have this as an option through Magers Health Center.
All of this is at no cost to university or member/dependent.
Get healthy, stay healthy
It is important that you get healthy and stay that way.
- Mercy will initiate a referral to Magers of members and/or dependents who disclose they have not seen a primary care physician in the last 12 months. In the event that a member or dependent affirmatively declines a referral to Magers, Mercy will offer a referral to a Mercy PCP.
- We want the healthy to stay healthy… so employees have the option of participating in the Health and Wellness Connection (“HAWC”) program at no cost to university or member/dependent.
- Mercy and MSU are in the process of developing onsite wellness educational classes that will be offered at no cost to university or member/dependent. Stay tuned!
- Lastly, employees have the option of speaking with their physician about the results and next steps.
