The Internal Revenue Service (IRS) requires employers to provide a Form 1095-C to full-time employees enrolled in the employer’s medical plan. The University will be sending all employees enrolled in the University’s medical plan during 2019 a Form 1095-C by March 2, 2020.
If you enrolled in Missouri State University’s medical plan for any coverage in 2019, the form provides the information needed to verify health coverage for your income tax return that you (and your spouse and dependents, if applicable) had qualifying health coverage for some or all months during the year.
- You do not have to wait on this form to file your taxes.
- Other information can also be used to verify health coverage. This can be: Insurance Cards, W-2 or payroll statements reflecting deductions, or other statements showing you or a family member had health care coverage.
