What is a Form 1095-C?

Form 1095-C provides essential information about the healthcare coverage offered or provided to you. This form may be useful when filing your tax return or determining eligibility for a premium tax credit.


Benefits of Receiving Form 1095-C Electronically

  1. Early Access:
    • Receive your form sooner than through the traditional mailing process.
    • A follow-up announcement will be made when the 2024 Form 1095-C is available for download.
    • To access the form electronically, you must opt-in before its availability.
  2. Convenient Access:
    • Print duplicate copies anytime throughout the year.
    • The electronic version contains the same information as the mailed copy.
  3. Cost Savings:
    • Reduces university expenses on printing and postage.

How to Receive Your Form 1095-C

  1. Log in to the My Missouri State Portal.
  2. Use the Discover Tool and search for “Employee Dashboard.”
  3. Navigate to the Employee Dashboard and follow these steps:
    • Select Taxes.
    • Click Electronic Regulatory Consent.
    • Choose Consent to Receive 1095-C Electronically.
    • Click Submit.
