Living with a chronic condition—or supporting someone who is—can feel overwhelming at times. The Chronic Disease Self-Management Virtual Program is designed to provide practical tools, education, and encouragement to help you feel more confident, informed, and in control of your health. We encourage you to take advantage of this meaningful, no-cost opportunity.
This evidence-based, six-week virtual program focuses on building skills you can use every day to better manage chronic health conditions and improve overall quality of life.
