As in previous years, Missouri State is offering employees hired before Sept. 1, 2011, the opportunity to offset the health insurance premium costs of $30 per month ($360/year) by participating in the Wellness Incentive Program. The Wellness Incentive Program includes:
- Biometric measurements and Health Risk Assessment ($20/month in savings)
- Tobacco usage questions ($10/month in savings)
This year Missouri State is using an online Health Risk Assessment (HRA) tool to collect your information—both the biometric measurements and the tobacco usage questions.
Biometric measurements
Providing your biometric measurements is the first step in completing the HRA. Biometric measurements include the following screenings:
- Cholesterol panel (total cholesterol, HDL, LDL and triglycerides)
- Blood sugar level
- Body mass index (BMI)
- Blood pressure
Biometric screenings can be conducted by staff at Taylor Health and Wellness, or you may also provide a signed copy of your biometrics from your personal physician, if the tests were conducted in 2011.
Free biometric screenings
Taylor Health and Wellness will be providing free biometric screenings for employees from Sept. 1-Oct. 7. You can schedule your appointment online through My Learning Connection, which can be accessed through the Professional Development channel on the Profile tab in My Missouri State.
Bring a completed Taylor Health and Wellness Center Release form with you to your appointment. Employees who complete their biometric screenings by Sept. 16 will be entered into a drawing for prizes, including an iPad and tickets to performances at Hammons Hall.
Health Risk Assessment and tobacco usage
Participation in the Health Risk Assessment begins with providing your biometric measurements, which are uploaded into the HRA. The HRA tool will be available for you to complete online from Oct. 24-Nov. 11. This tool includes various questions about your health and lifestyle, including tobacco usage.
Tobacco users who begin a tobacco cessation program by Dec. 1, 2011 and non-tobacco users who complete the HRA by Nov. 11 will not incur the $10/month health insurance premium in 2012.
Deadlines
To successfully receive the benefits of the Wellness Incentive Program, it is recommended that you complete the steps of the HRA by the dates outlined below:
- Provide biometric measurements by Oct. 7, 2011
- Complete the HRA by Nov. 11, 2011
- Begin a tobacco cessation program by Dec. 1, 2011 (if applicable)
