Missouri State University has completed its first year of reporting the ACA 1095 form to employees.  This requirement of the Affordable Care Act identifies those offered insurance coverage, as well as those covered.  New processes and forms were needed to comply with this federal reporting.  Brad Hamman, Systems Analyst, worked closely with Human Resources to clearly define the process.  Scott Turk, Database Analyst, enhanced the FormFusion process that populates the form with existing Banner data.  These are two examples of the many MSU staff contributions involved in the successful implementation of this project.
