The Internal Revenue Service (IRS) requires employers to provide a Form 1095-C to full-time employees offered and/or enrolled in the employer’s medical plan. 1095-C forms have been completed. Printed versions were mailed this week and electronic versions are currently available for print.
Access your Electronic 1095-C form:
- Go to your MyMissouri State portal
- Go to Profile
- select Employee Dashboard
- Taxes
- 1095-C Employer Provided Health Insurance Offer and Coverage Statement

Receive your form earlier next year by completing electronic enrollment
- Go to your MyMissouri State portal
- Go to Work Resources
- Under Time and Leave Resources,
- select Employee Dashboard
- Taxes
- Electronic Regulatory Consent
- Click “Consent to receive 1095-C electronically”
If you enrolled in Missouri State University’s medical plan for any coverage in 2021, the form provides the information needed to verify health coverage for your income tax return that you (and your spouse and dependents, if applicable) had qualifying health coverage for some or all months during the year.
- You do not have to wait on this form to file your taxes.
- Other information can also be used to verify health coverage. This can be: Insurance Cards, W-2 or payroll statements reflecting deductions, or other statements showing you or a family member had health care coverage.
