Open Enrollment starts right around the corner and there are new changes to take note of. In order to save $360 on your health insurance, you need to participate in a biometric screening instead of filling out the health risk assessment.

You need to turn in your biometric screening before December 1, 2019, to be eligible for the discount.

Here are the cliff notes:

1: If you’ve had your yearly physical already, print and complete this form. Make sure to send it to Mercy.

2: If you have not had your yearly physical OR you are missing required items, sign up for a FREE screening. Make sure to print out and bring this form!!

3: Access your biometrics online after your screening.
