• Oct. 30 – West Plains Student Recreation Center, 9 a.m.–1:30 p.m. (Magers, by appointment)
• Year-round – Magers Health and Wellness (by appointment, Jan. 1–Dec. 1)

🌿 Chronic Disease Self-Management Program

Take care of your health — and support family members or caregivers managing chronic conditions.

• 📅 When: Thursdays | Oct. 16 & Nov. 20 | 6:00 p.m.–8:00 p.m.
• 📍 Where: 3265 S. National Avenue, Springfield, MO
• 💲 Cost: Free (registration required)
• 📞 Register: Call 417-841-5667 by Friday, October 10

This six-week program is for anyone living with a chronic disease, as well as family members or caregivers. Participants will receive Living a Healthy Life with Chronic Conditions and learn important management skills.

💡 Wellness Incentive

Full-time employees enrolled in the Health Plan can earn up to $30 per month ($360 per year) by completing any two of the following by Dec. 1, 2025:

• Biometric screening
• Flu vaccination
• Educational questionnaire
• ✅ Each activity = $15/month
• ✅ Complete two = Full $30/month incentive
• ✅ Completing all three is encouraged (but the maximum incentive remains $30/month)
