We’re thrilled to introduce new electronic forms for updating employee names, addresses, and benefits related to qualifying life events. This enhancement will streamline the process, making it faster and more convenient for employees to manage their personal information and benefits.
Qualifying Life Event Updates:
Qualifying life events for benefit changes include situations like marriage, divorce, birth or adoption of a child, and significant changes in employment status. Employees have 31 days from the date of the life event to request changes and provide supporting documentation. Changes will be effective on the 1st of the month following receipt of the request and supporting documentation, except for new babies, whose coverage will be effective on the date of birth. Changes outside of this 31-day window can only be made during the open enrollment period, which runs from November 1 to December 1. Changes made during open enrollment will have an effective date of January 1. Access the Qualifying Life Event form.
Employee Address or Name Changes:
To update your employee address, or your legal name, please use the new electronic address/name change form.
Please note that international students and employees should continue to update personal information through the International Services Office, and student address changes should be made through the Registrar’s Office.
