Complete at least two activities — the educational questionnaire (Nov. 1–Dec. 1), a flu vaccination (Aug. 1–Dec. 1), or a biometric screening (Jan. 1–Dec. 1) — to earn your health plan discount by Dec. 1.- Biometric screenings must include A1C and Lipid panel results.
- If completed outside Magers Health & Wellness, submit records to MagersMedicalRecords@MissouriState.edu with the subject line “Flu Vaccination” or “Biometric Screening” as appropriate.
