Each February, the American Dental Association (ADA) sponsors National Children’s Dental Health Month to raise awareness about the importance of oral health. Developing good habits at an early age and scheduling regular dental visits helps children to get a good start on a lifetime of healthy teeth and gums.
The MSU sponsored benefits can help you and your eligible dependents gain and maintain good oral health.
Dental Benefits include:
- Preventative care coverage
- Covered Preventative services paid by the plan at 100%
- $3000 Yearly Benefit plan max
- Maximum is the amount that the plan will pay per person, per calendar year
- Orthodontia coverage for dependents
- Eligible dependent children under the age of 19
- 50% of eligible charges up to $3000 yearly max
- No lifetime orthodontia max
- $50 individual deductible / $150 family
- Coverage for Implants
- 50% of covered charges up to $3000 yearly max
- $50 individual deductible / $150 family
Save more by choosing an direct contract or in-network dental PPO participating provider and take advantage of contracted discounts. *Out of network dental providers have the same percentage reimbursement rate, but covered charges are based on usual and customary allowances. Amounts over usual and customary are non-covered and the provider may bill the patient for the excluded amount.
Medical Plan Benefits:
Dental related services under the medical plan are generally excluded. However, facility and anesthesia charges for pediatric or adult dental procedures that require the use of anesthesia in an Outpatient Surgical Center or Hospital setting for the following Covered Persons:
(i) A child under the age of five;
(ii) A person who is severely disabled; or
(iii) A person who has a medical or behavioral condition which requires hospitalization or general anesthesia when dental care is provided.
Physician’s charges for the dental procedure are not eligible under this Medical Plan unless stated as covered in the Medical Benefits above. Documentation of the Medical Necessity should be submitted with the charges. If the Covered Person has elected coverage under the Employer’s self- funded dental plan, anesthesia charges in the Physician’s office will first be eligible under the dental plan and then coordinate coverage under this medical Plan.
Flex Spending Benefits:
Dental related expenses including co-insurance and deductibles are eligible expenses under the Cafeteria/flex spending plan. Utilization of the cafeteria plan allows additional savings through use of pre-tax funds. Employees currently enrolled in the plan can submit eligible expenses to ASIFlex.
- Submit claims during the grace period to utilize remaining 2023 funds
Accident Plan Benefits:
Enrolled in the Allstate Accident plan? Did you know that the Physician’s treatment benefit of $50/$75 can be claimed and reimbursed directly to you? Services reimbursed under this portion of the plan benefit do not have to related to an accident. Payable physician treatment includes a dental exam; File a claim.
Enrollment/Changes can be made to the Accident, Medical, Dental and/or Cafeteria/flexible spending plans, yearly during open enrollment (November 1-December 1), or within 31 days of a qualifying event.
